Twist the plunger so that it touches the skin when the suction cup is applied. If you twist it too much - it will fall out and be returned by inserting and turning the tap clockwise. Turning clockwise moves the plunger away from the skin and creates a vacuum and vice versa. Use caution when using on another person. Larger suction cups may be painful with a larger vacuum. Use gel or shave the area to aid adhesion.
